I was talking to a man who I had admitted during my last night shift. He had come in to resus bleeding, with an unrecordable blood pressure and skin the colour of snow. I complimented his new, rosier complexion and said how pleased I was that he was well enough to leave the ward.
‘I have always wondered,’ he told me, ‘how people like you do your job, but I’ve been looking around the last few days and you’re all just so happy, you do this because you love it.’
So I think about the past few days:
It’s 2am and I am sat in a room with a daughter, her husband and her son. I tell her that the time has come to withdraw life-prolonging treatment on her mother and that I expected her to pass away very soon.
There are tears and the husband looks at his wife and says: ‘We need to let her go back where she came from, love.’ The daughter understands and she says ‘thank you’ and ‘it must be a very difficult thing to do to have to give people this sort of news’.
We left and the nurse said, I think you did a good job there.
It’s a day or two later and I have just helped a nurse pick a woman off the floor. She has fainted from the grief of being confronted with her moribund sister. Collapsed under the weight of it. Computer said no, I guess, but when she rebooted and opened her eyes, nothing had changed.
Then I am standing in a room with four sisters; one of them is brainstem dead, three of them are distraught. Their grief is intrusive, it is indiscreet and it is raw.
There is shouting; they shout at their sister and tell her to come back. Then they shout at me and say ‘come on, you have the machines, we just want you to try’ and I tell them there is nothing to be done and they shout louder, they beg and ask what they would have to give me for me to change my mind.
They ask if I know how important she is to them, I tell them I do and I tell them that there is nothing to be done. I don’t know if I feel sad in that moment, I think I feel empty.
I leave that room and a senior registrar is standing in the doctor’s office with coffee and a cake, for me and he says ‘you need this’.
Later I walk passed a senior nurse and she stops and asks if I am ok and when I am leaving work the consultant tells me, I was great today.
I work in a job where there can be so much sadness and I am at first incredulous that this patient can look at me and think I am ‘just so happy’. Jumping to the memories of human tragedy that peppered that week I realise that they are also memories of kindness.
That he looked at me and saw joy and vocation, I am proud but I know I am indebted to my colleagues; they balance the books, they make it OK.
