Five year forward view

What is the five year forward view?

The five year forward view (5YFV) is a wide-ranging strategy for the NHS in England that was published in October 2014.

It is referred to as being ‘by the NHS’ given the lead and partner organisations: NHS England; Care Quality Commission; Health Education England; Monitor; Public Health England; Trust Development Authority; and NICE. The government has given full support to its implementation, although they have not delivered on their promise to 'fully fund' the £10bn called for in the 5YFV.

The 5YFV covers a number of themes, such as the importance of public health and ill-health prevention, empowering patients and communities, strengthening primary care and making further efficiencies within the health service.

In March 2017, NHS England published Next steps on the Five Year Forward View. This plan takes stock of progress at the half way point of the five year forward view and sets out priorities for the next two years. The BMA has produced a paper highlighting the key areas of interest for doctors.

While many of the measures in the plan are positive, questions remain over the severe funding pressures within health and social care.


  • What are the new care models?

    The 5YFV proposed a number of new care models, designed to break down the traditional divides between primary, secondary and community care, mental health and possibly social care. The models are:

    1. Multispecialty community providers (MCPs)
    2. Primary and acute care systems (PACS)
    3. Urgent and emergency care networks
    4. Acute care collaboration
    5. Enhanced health in care homes


    How are the new care models being implemented?

    The models are being piloted through a national programme known as ‘the vanguard’. This consists of 50 sites across England, including nine PACS and 14 MCPs. For a list of and more information on the individual vanguard sites see the NHS England website.

    The aim is that vanguards will share their learning and make it possible for the models to be replicated more widely across the country. Many areas are now planning to develop one or more of the models through their STP (sustainability and transformation plan).

    We continue to press the point that the efficiencies required of the NHS in England - £22bn per year by 2020/21 – are unrealistic and unsustainable.
