Empowering parents to manage children's health

The Partnership, based in Tower Hamlets, London working together with its patients, developed a programme of support for parents with children under five years, to help them better manage self-limiting childhood problems. Analysis over a three month period showed that attendances to the GP, out-of-hours GP and A&E were reduced among the children of those parents who had attended the most sessions.

St Lawrence Surgery, West Worthing self-care strategy

Following consultation with practice staff and patients, the practice implemented a self-care strategy aimed at empowering patients to do more to self-care. Patients have access to web-based advice and assistance, bi-monthly newsletters to patients by email, and update on activities by social media. Implemented by the practice and requires no extra funding. Impact of the initiative is yet to be evaluated.
