3 - Miscellaneous work in the NHS

Within the remit of the Doctors and Dentists Review Body
Under their terms and conditions of service, hospital doctors can receive fees from their NHS trust for undertaking work related to their contractual duties provided it does not form part of their duties.

In addition, fees may be paid for radiology and pathology tests required as part of examinations and reports in connection with the routine screening of employee of health authorities, NHS trusts and local authorities (covered under individual terms and conditions of service). This applies to such cases where either time is not allocated to such work in a practitioner's contract, or the volume of work does not justify a separate arrangement.

The fees for this work are reviewed annually by the Review Body on Doctors and Dentists Remuneration (DDRB), which reports directly to the Prime Minister. A number of fees for work in the NHS which are included in the DDRB’s remit are given below. Please note that Fees guidance schedule 4 - go there gives the fees payable for family planning work where these fall within the remit of the DDRB.

1. Lectures to NHS staff

a) Lectures to non-medical staff given by:

i) Consultants and public health physicians
- per lecture : £56.87

ii) Associate specialists,
Senior registrars,
Specialist registrars at incremental point 3 or above,
Hospital practitioners,
Practitioners holding appointments under paragraph 94,
Senior clinical medical officers

- per lecture : £45.07

iii) Doctors in other grades
- per lecture : £33.09

b) Lectures to medical staff (as part of approved postgraduate medical education)
- per lecture : £72.05

2. Domiciliary consultations

a) Standard rate
- (first case per visit) : £78.38

b) Intermediate rate
- (second to fourth case per visit) : £39.19

c) Lower rate : £19.62

d) Series of visits in connection with anticoagulant therapy or treatment with cytotoxic drugs – maximum of : £235.14

e) Aborted visit (ie patient not at home at prearranged time): 50 per cent of the appropriate fee

f) Form CV1 – where it is completed in the course of or following a domiciliary consultation for hospital purposes without a further visit being necessary
- combined fee : £119.54
- re-examination fee (provided previous CV1 available) : £102.14

3. Exceptional consultations

a) Consultant : £146.85

b) General practitioner : £48.46

4. GP work in hospitals (£pa)

Where medical services are provided by general practitioners taking full clinical responsibility for their patients a ‘staff fund’ is created and shared among GP staff as they themselves determine. Payments made to the staff fund, which are payable under paragraphs 88–93 of the hospital terms of service, are:

a) Payment for each eligible bed : £624.00

b) Provision of a casualty service:
i) Flat rate:
- higher rate (24-hour service) : £7,691.00
- lower rate (12-hour service) : £3,846.00
- 12 hours per weekday : £2,750.00

plus

ii) Clinical work:
- per notional half day per annum : £4,375.00
- per hour or less per annum : £1,164.00
- per 1–2 hours per annum : £2,328.00

or

iii) Where number of casualties is less than 200 per annum, for each casualty seen : £25.13

5. Hospital appointments held by part-time practitioners (£pa)

- per weekly notional half day : £4,375.00

    - subject to a maximum (ie for 9 sessions) per annum of : £39,375.00
- 1 hour or less per week : £1,164.00
- 1–2 hours per week : £2,328.00

Note: These payments are made for part-time work in convalescent homes, GP maternity hospitals or other types of hospitals where no other rate is appropriate, including GP hospital units in respect of work not covered by payments into the staff fund (go to para 4 above).

6. Radiology and pathology tests

Tests in routine screening of employees
- per test : £3.44

7. National Blood Service (occasional work)

- per hour or part of an hour : £23.69
- maximum payment per session : £71.07

Note: The National Blood Service does not allow payment to general practitioners for medical information obtained about donors’ health.

8. Medical superintendent of psychiatric hospital

Allowance to whole or part-time clinician who holds medical superintendent posts : £4,903.22 (£pa)

Note: Payable under hospital terms of service paragraph 49b.

9. Advisory appointment committees

Consultants (hospital/public health) attending from outside the region (college or faculty nominees)
- per day : £122.70
- per half day : £61.35

Note: Whoever is responsible for setting up an AAC at a trust or health authority should arrange indemnity for members of the panel (letter (26.7.99) Julia Richard, NHSE to Douglas Bilton, CCSC).

10. Clinical complaints procedure

Consultants acting as a second opinion in stage 3 of the clinical complaints procedure (annex B to HC(88)37)
- per day : £187.47
- per half day : £93.74

11. Section 47 of part 5 of the Adults with Incapacity (Scotland) Act 2000

Fee for completion of assessment and certificate under parapgraph 9.5.1 of the terms and conditions of service : £101.00
